Yet another shifting thread lolz

K, perhaps you gents and ladies can shed some light on things. Just trying to get some additional opinions before I go replacing parts lol, or get an idea on what I need to replace.

Issue:
The car shifts fine when its off, when the car is on sometimes it shifts perfect however most of the time it either grinds or wont go in gear and I either have to wait a sec and try again or blip the throttle a little.

** I have had issues recently where it wouldnt go into gear at all to save its life, however I parked it and the prob went away after a few minutes and isnt present again for while this has only occured 2-3 times. **

Things I have done thus far:

Rebuilt my Master and slave cylinders. Bled the entire system, No leaks and the pedal has a good feel to it. Also you can see the slave move when the clutch is pressed moves around an inch or so.

Things I have not done...

When I rebuilt the engine 2 years ago I never replaced/rebuilt my transmission as far as I know its original.

I also did not install a new clutch at this time because the one on there looked good.

Things I believe could be the problem...

I need to adjust my clutch pedal??? (i doubt it is this as I feel the pressure engage about an 1.5 inches or more from the floor and the slave is moving. however I am welcome to try it again lol already adjusted it out once.)

My trans is shot... while I am sure I could use a rebuild/new one I doubt this is the prob...

My clutch is going out... I am inclined to beleive this since it feels like it grabs alot when I am going slower (such as pulling into/out of a parking spot where the speed is really low).

Thoughts..... or does everyone think I am on the money.
